Do you think the policy should depend on the work environment? Yes - what is appropriate in one setting may not be in another.
Do you think uniforms are a good form of dress code? Again, in certain settings it is appropriate. though the idea of having our whole office wearing uniforms is rather entertaining. :)
Do you think in an office setting with professional-type people that business casual is an appropriate dress code policy? Yes, for some people. I wear business casual pretty much every day because I do not work with outside clients - just internal partners. and even Avondjurken 2012 then, mostly via phone and email.
If you were to define "business casual" could you please define it for both men and women? No jeans, no t-shirts with printing/slogans, no tennis shoes or flip-flops. Nice pants/skirts and shirts with decent shoes, all taking modesty into consideration.

They can be, depending on the environment.

It can be, if it is well-defined and clearly communicated.

What I would consider a well-defined policy would be more than what I could fit here lol. My current organization's dress code includes a 3/4 page list of examples of appropriate business casual attire.
In essence, though, proper business casual attire is neat and professional shirts/blouses and slacks/skirts. Clothing should be clean, pressed, and in good condition (no holes, frayed edges, etc.). It should be loose-fitting and of an appropriate length for the office. Tank tops/spaghetti straps are only appropriate under a sweater or jacket. Denim is not business casual.
Take that FWIW, though, as I work in an industry (accounting) that is still very very conservative, and only relaxed standards from business professional dress every day recently. In fact, when Avondjurken 2012 I was originally interviewing for a job just out of college, one of the things one prospective employer made sure to mention was that women were allowed to wear pant suits at their firm!
